Book an appointment?

Same day Appointments –Please call the surgery if you require an appointment that day. A GP will call you back to discuss the nature of your problem and then book an appointment for you to see the most appropriate member of the team. Patients are encouraged to contact the surgery as early as possible for an appointment or visit.

Pre-bookable Appointments – All team members have routine appointments that can be booked up to 4 weeks in advance.

Telephone Consultations - an allocated time is given to the patient for the GP to contact them to discuss medication reviews, test results, or anything which does not require a physical examination.

Register?

To register, please pick up a patient registration pack from reception. These forms are also available online, below. An online service is coming soon. If you have any queries, any member of our staff will be happy to help you either at the surgeries or on the telephone.

Adult Registration - Please can you complete the following and return to the receptionist.

  1. New Adult Patient Letter
  2. New patient application form
  3. GMS1 Registration Form (purple form)
  4. Important information about your detailed Health records - Please Read
  5. Your Electronic Patient Record & the Sharing of Information - Please Complete
  6. Changes to your Records - Please read

Child Registration - Please can you complete the following and return to the receptionist.

  1. New Child Patient Information Letter
  2. GMS1 Registration Form
  3. Child Ethnic Origin Form
  4. Important information about your detailed Health records - Please Read
  5. Your Electronic Patient Record & the Sharing of Information - Please Complete

If you have any problem concerning your child’s health please contact out Health Visitors on 01924 224 299.

Request an ambulance?

If you require an emergency ambulance, dial 999.

Otherwise, you should ring the surgery on 01924 224 299. It should be noted that we require the following information:

  • The Hospital that you are going to
  • The Consultant / Doctor that you are seeing
  • The Date and Time of your appointment
  • Whether or not you are an In Patient or an Out Patient
  • Whether or nor not you require a car or and Ambulance
  • Whether or not you require an Escort
Obtain test results?

Please telephone after 11.00am for results of smear tests, X-rays or laboratory tests. Information cannot be given to another person unless we already have your express permission.

The practice will inform patients of any abnormal results that need to be acted upon.

Complete a form?

The doctor is able to sign and complete claims and forms on your behalf. However, this may have a charge associated with it. The reception team know the costs involved, so you can ask them or phone us. Many of these charges can often be claimed back from your insurance company or employer.

Examples of these forms are:

  • Private Sick Notes
  • Holiday Cancellation
  • Private Letters
  • Insurance Claims
  • Sickness / Accident Insurance Benefit Claims
  • Driving License Photographs
  • Freedom from Infection

Please leave these forms with the receptionist, who will pass them on to the doctor

NB: We are no longer able to complete Passport forms. All forms submitted will be sent straight back.
